Can I pick your brains please?

I missed the shortage notification informing us that Daktacort 2%+1% 30g cream was discontinued in April and stock will come to an end around May/June time.

I can see that the 15g is still available – are we able to prescribe 30g and dispense 2 x 15g?

With regard to the Daktacort, you need to be careful as the cost of the 15g tube is £4.96.

It will be ok to prescribe 15g as you will get reimbursed £4.96 but if you prescribe 30g with the intention of giving 2 tubes, the 30g prescription may only be reimbursed at £2.49 and the cost of meds will be 2 x £4.96 = £9.92 – thus making a loss.

This is a silly question that I probably should know the answer to but I needed to check with someone in the know !!

If the drug but not the pack size we are dispensing is in the Drug Tariff book should we be adding manufacture and price? For example Morphine solution-  only 300ml is in the Tariff  but we dispense 500ml & 100ml.  Also Terbinafine 250mg only has 14 listed in the Tariff but we dispense 28.  Colecaciferol 1,000 tabs pack size 28, capsules  pack size 30 but we dispense 28 & 30 for both  !!  I hope this makes sense?

Prescription services should work it all out pro-rata.

So for 28 Terbinafine, they will pay you £2.36 which is 2 x £1.18.

Or the Colecalciferol- if the tariff says 30 but you give 28, then you will get a slightly reduced amount reimbursed.

Please can you confirm if we should be outsourcing Mounjaro?

Mounjaro is classed as Personally administered but (correct me if I am wrong), is also dispensed and the patient takes away and injects themselves?

 

As long as the VAT is claimed correctly (same principle as an insulin) then the Mounjaro is profitable.

Just check with your finance team that that they are claiming the VAT correctly.

As long as it is, then fine to dispense in house.
